One such item that you will come across is Shark Fin, but the difficult part about acquiring a fin is to learn to make use of it. Where can you use the Shark Fin in Path of Exile 2?
You will need to interact with Kaimana, an NPC located at Kingsmarch, to use the Shark Fin.
You will get the Shark Fin by defeating the Great White One at Whakapanu Island. The item will be listed as Shark Fin: “The sea gave it strength. Now it serves a greater Purpose.” Once you interact with the Shark Fin on the ground, the quest “Tribal Medicine” will appear on the HUD.
The objective of Tribal Medicine will state: Find a use for the Shark Fin. Travel to Ngakanu, defeat the enemies, and rescue Kaimana. She will ask you to stop Tavakai, Tukohama Chieftain. After you defeat the Tavakai at the Heart of the Tribe, head to Kingsmarch town. Here you will find Kaimana near the docks. Talk to her and she will mention the Shark Fin that you received from the Whakapanu Island.

Rewards On Completing the Tribal Medicine Quest In Path of Exile 2
Kaimana will talk about using the Shark Fin to cure a blood fever. After you give her the fin, you will have to choose one of the three Uncut Support Gems:
- Uncut Skill Gem (Level 11): Creates a skill gem or levels an existing gem to level 11.
- Uncut Spirit Gem (Level 11): Creates a persistent buff skill gem or levels an existing gem to level 11.
- Uncut Support Gem (Level 4): Creates a support gem.
