How To Beat Patient 13 In BO6 Zombies
To spawn the final boss of Terminus; Patient 13, you must complete the Terminus Island main quest Easter egg in Black Ops 6 Zombies. But that isn’t the only thing you’re going to need, because some crucial items and perks must be gathered before the fight to make it easier. So before we dive into the strategies for each phase, let’s look at what preparation you need to do and what equipment you need.
Recommended Equipment And Preparations
Have a level 3 armor, a wonder weapon, and a self-revive kit at the bare minimum for the Terminus Island final boss fight. On top of that, you can also ensure you have a gold rarity or higher weapon by using an Arsenal Machine. Try to secure a Chopper Gunner if possible and don’t forget to have at least 10,000 Essence for buying ammo within the fight.

Phase 1 Strategy
During the first phase of this three-phase boss fight, Patient 13 will have huge glowing orange weak spots scattered around its body. Once you shoot off the first set of orange spots, the next ones will show up, and so on. Keep an eye on where the tentacles pop out from to avoid the Tentacle Slam attack. The tentacles always slam down right in front of the area they pop out so move away quickly. Patient 13 will also sweep these tentacles after slamming them so go as far as you can.
There will also be toxic tentacles that pop out further away and shoot long-range toxic attacks. You will have to shoot these ones to get rid of them, unlike the slamming tentacles. More specifically, you need to shoot the bright yellow spots on the toxic tentacles to kill them.
Zombies are also going to be a nuisance throughout this phase and hinder your attacks on Patient 13. While this is easily manageable with multiple players, you can use the Decoys or Cymbal Monkeys here to lure the zombies away if you really need to.
Phase 2 Strategy
The second phase of the Terminus final boss in BO6 Zombies is somewhat of a short transitional phase. It starts after Patient 13 goes under the water and emerges back up again all covered in lava. The open mouth will be the weak spot for this phase and you just need to shoot there until you go into the next phase.
Phase 3 Strategy
The Tentacle Slams will be back for this phase, but the weak spot that you need to shoot will now be the bluish-white glowing eyes on Patient 13. But dodging the tentacles will be harder now because the arena will be split into two. Remember that you can jump into the water and use the ladder to go to the other side of the arena.
The most dangerous thing in the whole Patient 13 boss fight in BO6 Zombies comes in phase 3 in the form of a Tongue Attack. During this, you will see the tongue stick out and try to catch any players it can. This attack will one-shot you no matter what so avoid getting hit by the tongue at all costs.
Another attack you need to avoid is the Shockwave, but this one is easy to avoid by simply diving into the water and coming back up after it’s over. Just like the tongue attack, this can also one-shot you but you will have a lot of time to prepare when Patient 13 starts charging up.
Finally, this is the best time to use the Chopper Gunner if you managed to get one as we recommended. This will allow you to dish out some heavy damage and be invulnerable for the duration of the Chopper Gunner. It’s the best way to finish off and defeat Patient 13 in Black Ops 6 Zombies and guarantee your victory when it gets to very low health.
